Laparoscopic Endometriotic Cystectomy
A minimally invasive surgery to remove endometriosis-related ovarian cysts (“chocolate cysts”) while preserving ovarian tissue.
Purpose
Laparoscopic endometriotic cystectomy is done when ovarian cysts caused by endometriosis may be affecting fertility or ovarian function. These cysts (commonly called “chocolate cysts”) can impact ovarian reserve and may interfere with ovulation or IVF planning.
Removes ovarian cysts caused by endometriosis (commonly called chocolate cysts) which can reduce ovarian reserve.
Helps preserve healthy ovarian tissue while removing the cyst wall.
Supports fertility planning by improving ovarian function in selected cases.

Procedure
The endometriotic cyst is removed laparoscopically by carefully excising the cyst wall while protecting healthy ovarian tissue.
Evaluation & Surgical Planning
The cyst size, symptoms, fertility goals, and ovarian reserve are reviewed to plan the safest approach.
Laparoscopic Access
Small incisions are made and a camera is inserted to visualize the ovaries and pelvic area clearly.
Cyst Excision (Cystectomy)
Procedure: Cyst wall is carefully removed while preserving healthy ovarian tissue.
Closure & Recovery
The area is checked for bleeding, incisions are closed, and post-op guidance is given for recovery and fertility planning.

Benefits and Evidence
Benefits
Reduces pelvic pain, restores ovulation, improves IVF success rates.
It can also support fertility planning when endometriosis-related cysts are impacting ovarian function.
Evidence
Cochrane Review 2014 shows cyst excision improves spontaneous conception and fertility outcomes compared to drainage alone.
